Our client, a leading safety-net healthcare provider with a Just Culture approach, is looking to recruit a passionate and detail-oriented Clinical Stroke/Sepsis Coordinator to join their dynamic team.
This role is all about ensuring excellence in stroke and sepsis care, from coordinating programs to analyzing data, while working under the guidance of the Chief Quality Officer and Stroke Medical Director. Your work will save lives, improve patient safety, and elevate the quality of care.
What You’ll Be Doing:
Program Leadership: Help maintain and grow the Primary Stroke Certification Program while guiding stroke and sepsis initiatives.
Data Champion: Collect, monitor, and analyze stroke and sepsis data for regulatory compliance with agencies like CMS, The Joint Commission, and Get With The Guidelines.
Educator & Advocate: Provide ongoing staff education and patient resources while promoting community awareness of stroke prevention and care.
Collaborator: Partner with interdisciplinary teams, EMS providers, and community organizations to enhance stroke and sepsis management.
Innovator: Lead performance improvement initiatives using the PDSA model to drive meaningful change.
Who Our Client Is Looking For:
– An RN with at least 3 years of clinical experience in ICU, ED, or Medical/Neurologic nursing, particularly with stroke and sepsis patient populations. The Stroke part is the key... they will train you on Sepsis if needed.
– Someone with strong expertise in quality improvement, care management, and data analysis related to stroke and sepsis.
– A team player who thrives in a fast-paced environment and loves making a difference through education, collaboration, and advocating for best practices.
