We’re seeking a Certified Diabetes Care & Education Specialist (CDCES) to help patients in the South Bronxnavigate the daily realities of diabetes — from carb counting and insulin timing to glucose graphs that sometimes look like abstract art.
This role sits at the intersection of clinical care and real life.
One moment you might be explaining why that “healthy smoothie” spiked someone’s glucose.
The next you’re helping a patient insert their first CGM sensor or walking them through insulin pump settings that suddenly make their numbers behave.
You’ll provide Diabetes Self-Management Education and Support (DSMES) and Medical Nutrition Therapy (MNT)to patients across the lifespan — from adolescents newly learning the ropes of Type 1 diabetes to adults finally gaining traction with long-standing Type 2.
And when someone proudly tells you, “My numbers finally make sense now,” you’ll know exactly why this work matters.
What You’ll Do
Your work centers on Diabetes Self-Management Education and Support and Medical Nutrition Therapy for patients with prediabetes, Type 1, and Type 2 diabetes.
In practice, that means helping patients:
• Understand CGM data and glucose trends
• Apply the ADCES 7 Self-Care Behaviors in everyday life
• Learn how to use continuous glucose monitors and insulin pumps with confidence
• Translate glucose data into practical nutrition and lifestyle strategies
You’ll also collaborate closely with endocrinologists, primary care providers, and the diabetes care team to help patients navigate complex chronic conditions while addressing barriers such as health literacy, technology access, and social determinants of health.
Qualifications
- Bachelor’s Degree in Food, Nutrition, Dietetics, or Nursing
- Registered Dietitian Nutritionist (RDN)
- Certified Diabetes Care and Education Specialist (CDCES)
- At least 2 years of experience as a Registered Dietitian
- Experience counseling children and adolescents preferred
- Current BCLS certification
- Familiarity with CGM and insulin pump technologies preferred
